Please remember to include the following in your licence application package:
√ A completed Application for Mechanical Licences Form
√ A completed Royalty Calculation Sheet (if using the PDF) or Print versions of the Application Form
√ A copy of either your pressing order, invoice, or letter from the presser confirming the total number of units to be manufactured
√ A completed, signed, and dated Pressing Information Waiver
And, if applicable:
√ Licence (from Other Collective) – if you entered a value in the “% Paid to Other Collective” field on the Application Form
√ Any agreements/authorizations received for adaptations/translations
